Population Health Essentials offers a 3-day comprehensive overview of population health and outlines the fundamentals necessary to improve outcomes and reduce costs.
Through case studies and real-life workforce applications, participants will review and analyze key areas of population health.

To successfully complete this activity, the required evaluation must be completed by the following dates. Thereafter, the evaluation will no longer be available and credits will not be issued.
Physicians/Nurses/Other Healthcare Professionals: The required evaluation must be completed by Thursday, June 17, 2021.
Pharmacists: The required evaluation must be completed by Wednesday, June 2, 2021. The shorter completion date is to allow for time to upload into CPD Monitor.
Clinicians, practitioners, administrators and executives from healthcare related industries, hospital systems, pharmaceutical and medical device companies, employers, insurers and consultants.
Following this educational activity, learners will be able to:
- Examine the role of key stakeholders and their complex interrelationships on population health outcomes in the US healthcare system.
- Describe the population health paradigm.
- Identify models and strategies for population health management and creating a culture of wellness.
- Examine the economic concept of risk as it applies to healthcare.
- Apply risk management theory to population health management.
- Discuss the impact that social determinants of health have on health outcomes .
- Discuss interventions that reduce the impacts of SDOH, improve health outcomes and decrease health care costs.
- Explain the necessity for data management and analytics for population health, in care delivery, care coordination, patient engagement and organizational
- Discuss the role of quality improvement in the context of population health management.
- Define models of care coordination and care management that proactively manage patient care across the continuum.
