The Population Health Essentials Workshop is an intensive professional development program that provides a comprehensive overview of the tenets of population health, including the key components of creating a population health model/framework.

In the U.S. and around the world, health and social care costs are growing at an unsustainable rate. Numerous sub-populations have unequal access to those services or, due to other social and economic factors, and experience poor and disparate health outcomes. Population health has emerged as a new framework for addressing health inequities and improving outcomes by integrating the fields of business management, quality, economics, informatics, epidemiology, health policy, and change management into a new paradigm of service delivery.

Attend the Population Health Essentials Workshop and learn how to help your organization, patients, clients address these issues.

Topics Covered: Overview of population health and value-based care, public health and the community, healthcare quality and safety, health economics, and population health intelligence.

Program Format: This 3-day multimodal educational program will encompass recorded and live didactic sessions, daily readings, discussion board participation and interactive live group sessions. Participants will be required to attend all live/on-line sessions as well as complete assignments outside of the live interactive sessions.
